Apply to Holy Disciples Catholic School
Holy Disciples Catholic School is a diverse community and we welcome your application. Please complete the online application form as a first step. Families are then encouraged to contact the school to arrange for a tour or a shadow date.
Upon receipt of the completed materials, a meeting will be scheduled to discuss any next steps and to finalize enrollment.

Application Process
Please complete the following steps to apply for the upcoming academic school year:
- Attend an Open House or Schedule a Tour
- Complete an online application
- Schedule an interview with the principal of the campus where you are applying for admission
- Complete all forms & documents listed in the Enrollment Checklist
- Submit the $250.00 non-refundable registration fee due per student
- Return additional required documents (Pre-K-8) (see below)
If entering Kindergarten, the child entering Kindergarten must be five years old by September 1st of the school year into which they are applying
Required Documents for Registration
- A copy of the most recent Report Card or Progress Report (PK)
- Birth Certificate (long form)
- Baptismal Certificate (if applicable)
- Standardized Test scores (if applicable)
- Current (within 12-23 months) Health Record signed by your pediatrician and including all immunizations
